Complete, sign, date, and send the Request for Replacement License- HTSB Form LA 6009 to HTSB.

Due to the large number of extension and renewal requests we receive every day, you may receive your replacement license about two to four months from the date HTSB receives your request.

If you find your license before you receive its replacement, please email licensingsection@htsb.org to cancel your request for a replacement license.
