Disciplinary Actions

The Hawaii Teacher Standards Board investigates violations of the HTSB Code of Ethics and Hawaii Administrative Rules §8-54-9.14 by licensees, permit holders, and applicants.

Based on the documentation and evidence presented at cases (including testimony from individuals), the Board may take action on a license, permit, or application.  This action may include, but is not limited to: suspension, revocation, or letter of reprimand.  In some cases no action is taken.

Report of Violation – to be completed by a school administrator

Report of Violation – to be completed by the general public
